To locate a report, you can search using any of these criteria:

• REPORT NUMBER – Enter the 12-character Crash / Incident / Arrest report number in this format: YYYYMMDD-###  (i.e. 20110301-123)
• DATE – Enter the date of the report in this format: MM/DD/YY (i.e. 03/01/11)
• NAME – Enter the last name of the reporting person (i.e. any driver for a Crash, Victim/Reporting Person for an Incident Report, Arrestee’s Name for Arrest Report.
• LOCATION – Enter the street name where the Crash, Incident or Arrest occurred.
(Note – you must enter a street name, not a road number)
